Accessing your Records

ESTABLISHED PATIENTS OF PLACER FAMILY MEDICINE

Your health records are available electronically without cost by visiting: myhealthonline.sutterhealth.org. Patients may inspect their records upon making a written request to our office and upon payment of reasonable clerical costs to make such records available.

Our office can provide you with a copy of your medical records for 25 cents per page plus clerical costs. Please allow our office fifteen (15) days to process your medical records request after we receive it.

You must submit a written request to access your medical records by emailing us at HELLO@PFM.HEALTH or sending a letter to 6620 Coyle Avenue, Ste 210, Carmichael, CA 95608. Once we have received a proper written request from you and we compile the records, we will then collect the money from you and provide a written statement.
