Billing

Placer Family Medicine accepts all of the insurances listed. All copays, coinsurances, and other fees that are not covered by your insurance are due at the time of your appointment.

  • And many more! Please complete the New Patient Intake Packet (NPIP) to initiate your insurance verification of eligibility and benefits.

Billing Inquiry Information

Patients must submit all billing inquiries in writing to billing@pfm.health. The turnaround time for processing billing inquiries is 2 weeks.
